At this time, it is my esteemed pleasure to officially announce the Polish Roman Catholic Union of America’s 59th Convention Special Life Insurance Plan. It is a new life insurance plan that has been specifically created to promote the 59th Quadrennial Convention.

The Polish Roman Catholic Union of America’s 59th Convention Special Life Insurance Plan provides a $59.00 voucher which will be credited to your initial premium as well as each of the additional annual premium payments for this life insurance plan. 	Download, print and fill out the life insurance application from the list below which is applicable for the state that you reside in and amount of life insurance coverage that you are purchasing with the 59th Convention Special*. These applications are provided in Adobe® Acrobat® format.
