TOURNAMENT ENTRY
All entries and applications for new members must be completed and returned to the Home Office by mail or email along with the $100 entry fee per team. Entries must be submitted by Friday, October 18, 2024. No entries will be accepted after the deadline.
All players must be PRCUA members in good standing. Each player may only sign one roster and must submit a Waiver of Liability form for 2024 (one per year). The waiver can be completed and submitted through DocuSign.
Players must agree to abide by the PRCUA rules and regulations of good sportsmanship and fair play on and off the playing fields.
Note: If you are submitting a check for this (or any other tournament), please make it payable to the Polish Roman Catholic Union of America. Due to changes in our bank’s procedures, we can no longer accept checks made payable any other way. Checks can be mailed to: PRCUA Fraternal Department 984 N Milwaukee Ave, Chicago IL 60642.
